RPM/CCM/PCM/APCM Care Coordinator of the Month: Dr. Shayma Mazumder
Dr. Shayma Mazumder is a pillar within the CCM/PCM programs. Her consistent reliability is readily recognized and appreciated by her teammates, who view her professionalism as a testament to her commitment—especially when it comes to her meticulous attention to detail.
“She consistently demonstrates strong organization and follow-through while managing the administrative and clinical coordination aspects of care management,” her program director explains. “Dr. Mazumder’s ability to maintain accurate documentation, coordinate patient needs, collaborate effectively with the team, and remain focused on quality reflects her dedication to both the department and the patients we serve.”
BHI/CoCM Care Coordinator of the Month: Alexis Talley
Alexis Talley brings noteworthy compassion and dedication to her role as a Care Coordinator. And the mother of two’s strong commitment to patient care soars above the status quo.
An LPN of five years now, Alexis exemplifies what it means to keep pushing and maturing within the team. “She continues to grow professionally, expand her skills, and take on new responsibilities while making a meaningful impact on the patients she serves,” her program director shares. “This recognition is well deserved, and we are excited to see her continue to grow with MedElite.”
